Patients come to the hospital and have no clue about their medical history. Although most hospitals and offices are now shifting to a computerized system; all the systems are not talking to each other. In other words the information your family doctor has may not be accessible to the doctor you are seeing in the hospital and vice versa. Sure we can call your doctor, but write your medical history down somewhere and take it with you wherever you go. Write down the list of medicines you are taking and keep that list updated. Keep a copy in your wallet, on your phone, your tablet, and in your glove compartment. That list can save your life especially in an emergency when we do not have a lot of time to call your doctor.
